Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnosis, and treatment/interventions for patients and support systems across inpatient and outpatient populations. Collaborate with the Inter/Multi-Disciplinary Team (IDT) to develop and implement evidence-informed person-centered care plans while respecting patient self-determination and adhering to NASW ethics. Reduce health disparities and barriers to care through advocacy and interventions grounded in life span development and neurobiology. Work closely with various departments throughout the health system to deliver care across the care continuum.
Required Qualifications
- Master's Degree - Social Worker
- Oregon Clinical Social Work Associate License within 60 days of hire
- Master's Degree Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License
- Washington Clinical Independent Social Worker License
- License in states wherever care is delivered
- 2 years Relevant Social Work experience. May include CSWA experience
Desired Qualifications
- 2 years - Clinical work experience within healthcare, hospice and/or bereavement
- 1 year All EMPL's: Care Management: Experience in acute care settings
