Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnoses, and treatment interventions for patients and support systems across inpatient and outpatient populations. Collaborate with patients and the Inter/Multi-Disciplinary Team to develop and implement evidence-informed person-centered care plans. Perform duties that respect patient self-determination, reduce health disparities, and adhere to the NASW code of ethics and Standards of Practice. Work closely with various departments throughout the PHS to deliver care across the care continuum.
Required Qualifications
- Master's Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License upon hire
- License in states wherever care is delivered
- 2 years - Relevant Social Work experience
- Master's Degree Social Worker
- Within 60 days of hire Oregon Clinical Social Work Associate License
Desired Qualifications
- 1 year - All EMPL's: Care Management: Experience in acute care settings
- 2 years Clinical work experience within healthcare, hospice and/or bereavement
