Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnosis, and treatment/interventions/advocacy for patients and support systems. Collaborate with patients, support systems, and the Inter/Multi-Disciplinary Team to develop and implement evidence-informed person-centered care plans across the care continuum. Work closely with various departments throughout the facility serving all inpatient or outpatient populations. Perform duties that respect patient self-determination, reduce health disparities, and adhere to the NASW code of ethics and Standards of Practice.
Required Qualifications
- Master's Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License upon hire
- License in states wherever care is delivered
- 2 years - Relevant Social Work experience. May include CSWA experience
- Master's Degree Social Worker
- Within 60 days of hire Oregon Clinical Social Work Associate License
Desired Qualifications
- 1 year - All EMPL's: Care Management: Experience in acute care settings
- 2 years Clinical work experience within healthcare, hospice and/or bereavement
