Social Worker, MSW - $5,000.00 sign on bonus

Provide direct social work case management services to participants in the PACE program, including participation on the interdisciplinary team, home visits to assess safety and living environment, intake and family meetings, development and implementation of QAPI activities, and coordination of care including discharge planning. Offer individual and family counseling, crisis intervention, advocacy, and liaison with external agencies; maintain comprehensive case management records and assist with money management and financial eligibility. The role requires a Master’s degree in social work, a valid driver’s license, and experience with an elderly population; bilingual English/Spanish is preferred. This is a full-time position based in San Marcos, CA with no on-call time and a supportive, mission-driven team environment.
