Job Summary
Conduct comprehensive social work assessments, depression screenings, and quality-of-life evaluations for dialysis patients to develop individualized care plans. Provide patient advocacy, counseling, and emotional support while coordinating services with physicians, families, and external agencies. Screen for palliative care needs and assist with transportation logistics for dialysis appointments. Participate in interdisciplinary team meetings, unit huddles, and quality assessment initiatives to improve patient outcomes and transplant rates. Deliver education to patients and staff on social work topics and serve as a resource for employee assistance programs. Perform relief duties, project assignments, and committee responsibilities as required.
Required Qualifications
- Master of Social Work (MSW) degree from a school of social work accredited by the Council of Social Work Education
- Washington State Independent Clinical (LICSW) or Advanced Social Worker (LASW) license
- Independent Clinical Associate or Advanced Associate license
- Valid Washington State Driver's License
- Car insurance
- Full time (1.0FTE)
