Job Summary
Join our innovative hospice and palliative care programs, providing psychosocial support and social work services to patients and families in various settings. As a key member of the hospice IDG team, you will develop care and discharge plans tailored to patient needs.
Required Qualifications
- Graduate from an accredited School of Social Work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Driver's license preferred
Desired Qualifications
- Knowledge of Word and Excel preferred
- Ability to work within an interdisciplinary team environment
- Possess excellent organizational, communication and listening skills
- Ability to input clinical data proficiently
