Job Summary
Provide psychosocial support and social work services to hospice patients and their family members in inpatient units, nursing homes, or home care settings. Develop comprehensive social work plans of care, set goals, and make recommendations based on patient and family needs while maintaining accurate documentation of assessments and interventions. Serve as a social work member of the hospice IDG, acting as a liaison between hospice, inpatient, and nursing home staff. Participate in discharge planning for patients admitted to the inpatient facility and deliver pre- and post-bereavement services. Maintain New York State MSW licensure and graduate from an accredited school of social work.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Car and driver license required
Desired Qualifications
- Knowledge of Word and excel preferred
