Job Summary
Provide psychosocial support and social work services to hospice patients and their families in inpatient, nursing home, or home care settings. Develop social work plans of care, set goals, and make recommendations based on patient and family needs. Maintain accurate documentation reflecting palliative assessments and interventions. Serve as a social work member of hospice IDG and act as a liaison between hospice, inpatient, and nursing home staff. Participate in discharge planning for patients admitted to the inpatient facility. Offer pre- and post-bereavement services to support families navigating life-limiting illness.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Car and driver license required
Desired Qualifications
- Knowledge of Word and excel preferred
