Social Worker

Provide psychosocial support and social work services to hospice patients and family members in inpatient units, nursing homes, or home care settings. Develop social work plans of care, set goals, and make recommendations based on patient and family needs. Maintain accurate documentation reflecting palliative social work assessments and interventions. Serve as a social work member of hospice IDG and act as a liaison between hospice, inpatient, and nursing home staff. Participate in discharge planning for patients admitted to inpatient facilities. Deliver pre- and post-bereavement services. This role requires New York State MSW licensure, a graduate degree from an accredited school of social work, and at least one year of healthcare social work experience. Candidates must possess excellent communication skills, proficiency in clinical data entry, and a valid driver's license.
