Job Summary
Provide psychosocial support and social work services to hospice patients and family members in inpatient units, nursing homes, or home care settings. Develop social work plans of care, set goals, and make recommendations based on patient and family needs. Maintain accurate documentation reflecting palliative assessments and interventions, and participate in discharge planning for inpatient facility admissions. Offer pre and post bereavement services while serving as a liaison between hospice, inpatient, and nursing home staff within an interdisciplinary team. Requires New York State MSW licensure, one year of healthcare social work experience, and proficiency in clinical data input.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Car and driver license required
Desired Qualifications
- Knowledge of Word and excel preferred
