Job Summary
Provide psychosocial support and social work services to hospice patients and family members in inpatient, nursing home, or home care settings. Develop social work plans of care, set goals, and make recommendations based on patient needs. Maintain accurate documentation of assessments and interventions, and participate in discharge planning for inpatient facility admissions. Serve as a social work member of hospice IDG and act as a liaison between hospice, inpatient, and nursing home staff. Deliver pre- and post-bereavement services while working within an interdisciplinary team environment.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Car and driver license required
Desired Qualifications
- Knowledge of Word and excel preferred
