Job Summary
Provide psychosocial support and social work services to hospice patients and family members in inpatient, nursing home, or home care settings. Develop social work plans of care, set goals, and make recommendations based on patient needs while maintaining accurate documentation of assessments and interventions. Serve as a social work member of hospice IDG and act as a liaison between hospice, inpatient, and nursing home staff. Participate in discharge planning for patients admitted to the inpatient facility and deliver pre- and post-bereavement services. Requires New York State MSW licensure, one year of healthcare social work experience, and proficiency in clinical data entry.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Ability to work within an interdisciplinary team environment
- Possess excellent organizational, communication and listening skills
- Ability to input clinical data proficiently
- Car and driver license required
Desired Qualifications
- Knowledge of Word and excel preferred
