Job Summary
Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care. Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources. Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources. Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care. Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.
Required Qualifications
- Certified Social Worker (CSW) in Nebraska
- Minimum of a bachelor's degree in Social Work
- Basic Life Support (BLS) from the American Heart Association
