Job Summary
Social Worker (MSW/LSW/LISW) at McGregor PACE to manage a caseload of 55-62 seniors as part of an interdisciplinary team. Conduct semi/annual assessments, collaborate with nursing partners, perform cognitive and psychosocial assessments, plan interventions, provide counseling to participants and families, coordinate with community agencies, discharge planning, and participate in data collection and performance-improvement initiatives. Hybrid schedule: Monday–Friday, 2 days per week from home; no weekends or holidays.
