Job Summary
Coordinate patient treatment plans including family education, discharge planning, and follow-up. Conduct psychosocial assessments and group sessions to meet active treatment requirements. Participate in interdisciplinary meetings to identify problems, establish goals, and contribute to the Master Treatment Plan. Perform admission and discharge CQI data collection for assigned patients. Maintain productivity while adhering to DCH behavioral standards, safety protocols, and employee handbook compliance. Requires MSW degree, LCSE license preferred, and two years of inpatient hospital experience. Full-time onsite role with physical demands including lifting 50 pounds and emergency mobility.
Required Qualifications
- MSW in Social Work
- LCSE (Licensed Certified Social Worker)
- Masters Degree in Counseling
- Masters Degree in Psychology
- Masters Degree in Marriage and Family Therapy
- License or Certification as required by discipline
- Two (2) years of inpatient hospital experience

- Hearing and vision must be normal or corrected within normal range
- Ability to tolerate prolonged periods of standing and walking
- Ability to lift or carry 50 pounds

- Ability to push stretcher/wheelchair with average adult patient
- Ability to do frequent stooping and on occasion crouching, crawling or kneeling
- Ability to reach reasonable distances in any direction
- Ability to run in emergency situations
- Must have ability to feel and perceive temperature, texture, shape and size with fingertips
- Physical presence onsite
Desired Qualifications
- LCSE (Licensed Certified Social Worker) preferred
- Two (2) years of inpatient hospital experience is desirable
