Social Worker

Be responsible for addressing the social service needs of One Care members; provide follow-up sessions with health center clients or coordinating services by phone; conduct brief counseling, advocacy, & resource referrals; participate in case conferences; conduct home visits solo or with interdisciplinary team members; be an integral member of the primary care/BH team; advocate for patients with outside agencies; provide crisis intervention as needed. Successful applicant will offer human services experience; ability to work with children, adults, and the elderly; LSW required; minimum bachelor’s degree in human services; bilingual/bicultural candidates strongly desired; history of good attendance and positive work attitude.
