Job Summary
Assess patient social services and discharge planning needs, including post-discharge ability to provide for those needs. Identify situations indicating possible abuse, dependencies, or growth and development problems, then report cases to appropriate community agencies. Provide support to patients and families coping with illness or hospitalization while following regulations for post-acute placement. Make recommendations regarding patient needs and refer them to community or other appropriate services. Document all assessments, referrals, and interventions in the medical record. Maintain professional relationships with post-acute facilities and attend multi-disciplinary rounds to support length of stay goals. Attends family meetings when requested and works with nursing, ancillary, and medical staff with high professionalism. Requires NJ or LCSW license, Master's degree, and internship; salary $64,350-$102,960 annually.
Required Qualifications
- Master's degree in Social Work or in the related fields of Guidance and Counseling, Human Services, Marriage and Family Counseling, Psychology, Sociology, and Vocational/Disability Rehabilitation, from an accredited college or university
- Internship
- NJ Licensed Social Worker or Licensed Certified Social Worker
- Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team
