Social Worker LSW / MSW

Assess patient social services and discharge planning needs, including post-discharge ability to provide for those needs. Identify situations indicating possible abuse, dependencies, or growth and development problems, then report cases to appropriate community agencies. Provide support to patients and families coping with illness or hospitalization while following regulations for post-acute placement. Make recommendations regarding patient needs and refer individuals to community services. Document all assessments, referrals, and interventions in the medical record. Maintain professional relationships with post-acute facilities and attend multi-disciplinary rounds to support length of stay goals.
