Social Worker (LSW) - Minnesota Aging Pathways Resource Coordinator

Conduct person-centered support for older adults and caregivers by completing in-person and phone assessments, offering counseling on long-term care services, benefits, housing, and financial resources, and assisting with care planning and transitions following hospital or rehabilitation stays. Coordinate services and referrals with healthcare providers and community agencies, advocate for clients to ensure access to appropriate supports, and manage an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living. This role works Monday through Friday, 8:00 AM to 4:30 PM, with two in-office days per week and the remainder spent working from home or in the community. Employees must demonstrate high-speed internet connectivity and meet privacy and security standards when working offsite. The organization advocates for older adults by building age- and dementia-friendly networks to reduce care disparities and ensure access to essential services.
