Job Summary
Provide assessments and develop individualized treatment plans for patients and residents; deliver counseling, education, and support to individuals and families; facilitate group therapy sessions and support groups; collaborate with interdisciplinary teams to ensure comprehensive care; document patient and resident progress and maintain accurate records; develop and implement discharge plans in collaboration with the team; provide referrals to community resources and services; participate in quality improvement initiatives and continuous education activities.
Required Qualifications
- Bachelor's or Master's degree in Social Work (MSW) from an accredited program
- Strong communication, interpersonal, and organizational skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Compassionate and empathetic approach to patient care
