Job Summary
Social Worker (LSW/LCSW) – Home Services position based in Urbana, IL. Provides psychosocial assessments of patients and families, identifies emotional, social, and environmental needs, develops care plans including bereavement support, and communicates cultural and spiritual considerations to the interdisciplinary team. Responsibilities include educating patients/families to overcome barriers, documenting assessments and MSW interventions in the electronic medical record, implementing and evaluating social work plans to enhance family and patient coping, assisting with advanced directives, and supporting transportation/referral needs and access to community resources. Eligible for sign-on bonus; compensation range $27.36–$45.69 per hour with full benefits; role requires collaboration with an interdisciplinary team and adherence to palliative/hospice principles.
Required Qualifications
- Master’s Degree in Social Work
- Licensure/Certification Requirements: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) per IDFPR
- Driver's License valid with SOS
- Proof of Auto Insurance
- Experience: Hospice 1+ years
- Ability to work as part of a team
