Job Summary
Conduct psychosocial assessments of patients and families to identify strengths, problems, and cultural needs related to diagnosis and treatment. Develop bereavement plans of care and implement social work interventions that enhance family systems and community resource utilization. Interpret faith traditions and communicate spiritual beliefs impacting care, while educating patients on advanced directives like living wills and POLST forms. Document assessments, financial evaluations, and MSW interventions in electronic medical records and complete visit notes in a timely manner. Address imminent death indicators and facilitate self-determined life care decisions in collaboration with interdisciplinary teams. Assist with transportation barriers, referrals for basic needs, and applications for Medicaid or SSDI. Requires 1+ years of hospice experience; sign-on bonus available.
Required Qualifications
- Hospice experience of 1+ years
- Master's Degree in Social Work
- Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Driver's License - Secretary of State (SOS)
- Proof of Auto Insurance - Varies
