Job Summary
Conduct psychosocial assessments, trauma-informed crisis intervention, and field responses into the community to provide mobile mental health services within the C-U metro area. Respond to trauma, violence victims, and mental health patients in the ED, coordinating with security and EMS to notify next of kin and connect victims with community resources. Evaluate patients for abuse or neglect, make mandated reports, and assist with case planning for issues including SANE patients, chemical dependency, and guardianship. Arrange transportation, medication assistance, and housing resources to facilitate timely discharge and follow-up care. Attend leadership meetings to build communication bridges with the community and regional resources.
Required Qualifications
- Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Bachelor's Degree: Social Work
- Master's Degree: Social Work
