Social Worker LSW CSW

Conduct social service assessments and document findings according to state and federal regulations. Provide psychosocial assessments and relevant portions of the Minimum Data Set (MDS). Counsel residents while maintaining a team-oriented environment focused on resident and staff comfort. Gerontology or nursing home experience is preferred. This role supports St Martha Center's mission to make residents and staff as comfortable and happy as possible within an exceptional facility.
