Conduct discharge planning and assist patients and families with resource procurement and educational materials. Identify and analyze social and emotional factors underlying patient illness to collaborate with the interdisciplinary team on interventions. Maintain effective working relationships with internal and external clients while navigating personal and environmental difficulties affecting medical care. Spend 50% of time sitting and 50% standing or walking in a standard office environment serving a culturally and linguistically diverse population.
