Job Summary
Conduct thorough psychosocial, spiritual, and pre-bereavement assessments for patients and families while providing ongoing emotional support and counseling. Identify community resources, including financial assistance, and guide families through advanced directives and facility placements. Maintain accurate clinical documentation at residences and in agency records, actively contributing to interdisciplinary team meetings to shape holistic care plans. Manage caseloads while ensuring compliance with local, state, and federal laws and regulations. Participate in on-call duties on evenings and weekends as needed. Work Mon-Fri 8am-5pm within Bay, Saginaw, Arenac, and Midland Counties.
Required Qualifications
- Master's degree from school accredited by the Council on Social Work Education required in states as specified
- Licensure and/or certification as required by the State
- Minimum of 2+ years social service experience in a health care setting or social services agency
- Valid driver's license
- Willingness to travel locally for patient visits
- Self-directed with the ability to work with little supervision

- Experience in hospice, palliative care, or a related healthcare setting preferred
- Deep comfort with grief, loss, and end-of-life conversations
- Strong knowledge of community resources and care coordination
- Excellent documentation skills and attention to compliance
