Job Summary
The transplant social worker provides psychosocial care across inpatient and outpatient settings for heart failure, ventricular assist devices, and heart transplantation. Responsibilities span initial evaluation through candidacy review, waitlist management, transplant hospitalization, and lifelong post-transplant follow-up, including addressing clinical, emotional, logistical, and caregiver needs; helping patients navigate insurance, disability, and financial resources with clarity and empathy. Requires advanced clinical judgment, efficient decision-making under pressure, and the ability to perform urgent evaluations within a collaborative transplant program. Works as an essential clinical partner within a multidisciplinary team to support patients and families through complex care moments in heart transplant and LVAD care at Mayo Clinic Arizona. Visa sponsorship is limited to TN classification; Mayo Clinic does not participate in F-1 STEM OPT.
Required Qualifications
- LMSW licensure
- Social work experience in transplant or related clinical setting
- Strong psychosocial assessment skills
- Ability to manage high-stakes evaluations
- Knowledge of insurance, disability, and financial assistance resources
