Job Summary
Hospice Social Worker with an MSW/LMSW credential to join an interdisciplinary team delivering compassionate psychosocial support in home-based end-of-life care. Responsibilities include completing comprehensive assessments within 5 days of hospice admission, developing and implementing Plan of Care interventions, providing counseling on end-of-life issues, assisting with advance directives and discharge planning, coordinating community resources, collaborating with the IDG, and maintaining timely clinical documentation. Requires strong hospice knowledge, patient advocacy, caregiver support, and ability to work in the home setting; CPR certification and driving/auto insurance are required. Preference for HomeCare HomeBase experience. Location: Scottsdale, AZ (Haven Scottsdale Hospice Branch).
Required Qualifications
- Master’s Degree in Social Work (MSW) from an accredited program
- Current Social Worker License
- Minimum 1 year of Hospice Social Work experience required
- Strong understanding of hospice philosophy and end-of-life care
- Valid Driver’s License and current Auto Insurance
- Current AHA CPR Certification
- HomeCare HomeBase experience preferred
