Job Summary
Hospice Social Worker (LMSW) PRN in Las Vegas, NM with relocation assistance. Responsibilities include psychosocial assessment and plan-of-care documentation for patients and families in hospice, counseling and support for coping with stress, liaison with community resources, and assisting with applications for benefits (SSI, Hospice, Medicaid, Medicaid Waiver). Requires MSW and NM licensure (LMSW/LCSW) with at least one year of health care or hospice experience. Monday–Friday with flexible scheduling; competitive benefits including medical/dental/vision, 401(k), PTO, tuition reimbursement, Hospice Certification bonus, and salary increases based on performance. Required Qualifications
- Social Work Degree
- New Mexico Master7s Degree in Social Work required
- Current State Licensure in Social Work
- New Mexico: LMSW or LCSW
- Minimum of one year7s experience in health care or hospice setting
