Social Worker for Liver Transplant care at Virtua Lourdes Health System. Provides ongoing psychosocial assessment and counseling to patients with advanced organ disease, transplant candidates and recipients, living donors, and caregivers across inpatient and outpatient settings. Collaborates with multidisciplinary teams, conducts comprehensive psychosocial assessments, supports discharge planning, and coordinates access to transplant services. Requires MSW, LCSW preferred, NJ license, strong communication and crisis-intervention skills, and ability to navigate complex care coordination across transplant-care continuum.
