Social Worker - LISW/LCSW

Social Worker - LISW/LCSW needed to provide psychosocial, psychotherapy, and counseling services to patients across Georgia via telehealth from home. Full-time role (8:00am-5:00pm, Mon-Fri) serving Macon, Augusta, Savannah, and Brunswick areas. Responsibilities include conducting comprehensive psychosocial assessments, screening for mental health and social determinants of health, delivering individual/couples/family/group therapy,upport during acute crises (suicidal ideation, self-harm), developing and implementing care plans, documenting care management, integrating behavioral health into primary care, coordinating with care teams, educating patients and families about conditions and resources, advocating for patients, and maintaining confidential records. Qualifications require a Master’s Degree with licensure (LMSW or LISW-CP), licensed to practice Social Work, and preferred experience or certification in Substance Use Disorder treatment. Proficient in telehealth tools and electronic medical records; strong communication, leadership, and teamwork skills; ability to travel between offices.
