Job Summary
Plan, develop, organize, implement, and evaluate social services programs while developing social histories and assessments for residents. Coordinate department policies, administer procedures, and make regulatory recommendations to administrators. Participate in community planning, answer admission inquiries, provide tours to prospective families, and assist in updating the admissions waiting list. Conduct private interviews with residents and families, develop care plans with specific goals, and handle emergency rescheduling of work assignments. Submit required forms and reports as mandated.
Required Qualifications
- Bachelor's degree from an accredited school of Social Work or related field
- any certifications/licensures as required by the State of employment to practice in long-term care
