Job Summary
Assess, plan, implement, and review care and support arrangements for adults with learning disabilities while safeguarding vulnerable individuals and promoting independence. Complete strengths-based assessments, risk evaluations, and support planning in line with statutory requirements, then develop and monitor care plans collaboratively with families and partner agencies. Manage complex caseloads using professional judgement, undertake safeguarding investigations, and prepare reports for decision-making forums. Maintain accurate case records, evaluate commissioned services, and participate in duty rotas and supervision processes within budget frameworks.
Required Qualifications
- Social Care Wales Registration
- Enhanced DBS Check
- A recognised Social Work qualification (Degree, Dip SW, CQSW or equivalent)
- Experience within Adult Social Care, ideally working with adults with learning disabilities
- Strong assessment, care management, and safeguarding experience
- Sound knowledge of relevant legislation, policy, and best practice guidance
- Excellent communication, organisational, and case management skills
- An Enhanced DBS check or willingness to obtain one
