Job Summary
Social Worker in Learning Disabilities Team focusing on transitioning young people aged 17–25 from Children’s Services to Adult Social Care. Responsibilities include completing Care Act assessments and developing outcome-focused support plans, undertaking prevention and enablement work, Mental Capacity Act assessments, Best Interests determinations and DoLS applications, responding to safeguarding concerns and leading Section 42 enquiries, applying Making Safeguarding Personal principles, employing a strengths-based approach, participating in Pathway Plans, EHCP reviews and IRO meetings, exploring funding options including Continuing Healthcare and Section 117 aftercare, developing effective partnerships for smooth transitions, managing a diverse caseload, and maintaining accurate records including assessments, case notes and funding applications.
Required Qualifications
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
