Social Worker - Learning Disabilities

Job Summary

Undertake assessments, reviews, and care planning for individuals with learning disabilities, applying strengths-based approaches to promote independence and inclusion. Manage complex case work within a Learning Disabilities service, including risk assessment, safeguarding, and multi-agency collaboration with health professionals. Develop person-centred support plans, coordinate care packages, and produce accurate case recordings while ensuring compliance with the Care Act 2014, Mental Capacity Act 2005, and Human Rights legislation. Contribute to multi-disciplinary meetings and decision-making processes across rural and urban areas in Louth, Lincolnshire.

Required Qualifications

  • MA, BA or BSc in Social Work, Diploma in Social Work, or CQSW
  • Must be registered with Social Work England
  • Minimum 2+ years' post-qualified experience
  • Experience within Learning Disabilities and/or Adult Social Care
  • Strong experience in assessment, care planning and reviews
  • Experience of managing complex case work
  • Good legal literacy within adult social care
  • Ability to manage a busy caseload and prioritise work effectively
  • Experience working within a strengths-based, person-centred framework
  • Experience of multi-agency working and partnership collaboration
  • Evidence of ongoing professional development
  • Strong working knowledge of: Care Act 2014
  • Strong working knowledge of: Mental Capacity Act 2005 (MCA)
  • Strong working knowledge of: Human Rights legislation
  • Strong working knowledge of: Adult Safeguarding frameworks and procedures
  • Strong assessment and analytical skills
  • Excellent recording and report-writing ability
  • Ability to apply strengths-based and outcome-focused approaches
  • Excellent communication and interpersonal skills
  • Strong organisational and time management skills
  • Ability to manage complex workloads and competing priorities
  • Ability to work independently and as part of a team
  • Commitment to promoting independence, inclusion and choice
  • Must be able to lift 50 lbs
