Job Summary
Coordinate Social Worker schedules and ensure coverage across clinical locations while providing direct client support, including education, referrals, and brief case management. Lead monthly team meetings, guide Social Workers through workflows and referrals, and troubleshoot patient escalations in partnership with the Director. Maintain case documentation standards, implement workflow improvements, and represent the department organization-wide. Travel between sites as needed to support consistent access to services.
Required Qualifications
- Bachelor's degree in Social Work or a related human services field
- Excellent interpersonal and communication skills
- Ability to maintain confidentiality in all circumstances
- Strong computer literacy
- Ability to work independently and manage multiple priorities
- A client- and community-service mindset
- Ability to effectively lead groups and team meetings
- Strong problem-solving skills and the ability to respond effectively to both internal and external needs
Desired Qualifications
- Previous supervisory or team-lead experience
