Social Worker (LCSW) - Co-Responder Social Service Unit

Listen to presenting issues, apply motivational interviewing skills, and use crisis intervention to ensure safety and determine appropriate cause of action. Exercise clinical judgment to reach a resolution that is clinically appropriate and least restrictive while recognizing cultural beliefs of clients and families. Work with law enforcement, emergency departments, and other emergency personnel in crisis situations, providing follow-up case management as assigned. Maintain appropriate boundaries with clients and family members, and complete all documentation and forms of communication in a timely manner. Demonstrate understanding of Medicare Home Care benefit eligibility and services available, monitoring progress toward treatment goals by evaluating and adjusting treatment provided. Prepare and maintain pertinent records, statistics, and progress notes in the patient medical record, ensuring accurate and timely documentation. Provide culturally sensitive counseling and treatment while assisting in life crisis situations.
