Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnoses, and interventions for patients and support systems across inpatient and outpatient populations. Collaborate with the Inter/Multi-Disciplinary Team to develop and implement evidence-informed, person-centered care plans while respecting patient self-determination and adhering to the NASW code of ethics. Reduce health disparities and barriers to care by working closely with various departments throughout Providence. Perform duties that incorporate life span development, neurobiology, and behavioral change concepts within the PACE Oregon clinical program.
Required Qualifications
- Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License
- Washington Clinical Independent Social Worker License
- License in states wherever care is delivered
- 2 years relevant Social Work experience
