Job Summary
Conduct comprehensive biopsychosocial/spiritual assessments, diagnoses, and treatment interventions for patients and support systems across inpatient and outpatient populations. Collaborate with the Inter/Multi-Disciplinary Team to develop and implement evidence-informed, person-centered care plans while respecting patient self-determination and adhering to the NASW code of ethics. Reduce health disparities and barriers to care by working closely with various departments throughout PHS. Requires a Master's Degree in Social Work from a CSWE-accredited school, an active Oregon or Washington Clinical Social Worker license, and two years of relevant experience.
Required Qualifications
- Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License
- Washington Clinical Independent Social Worker License
- License in states wherever care is delivered
- 2 years relevant Social Work experience
- May include CSWA experience
