Job Summary
Conduct comprehensive biopsychosocial, spiritual assessments, diagnosis, and treatment interventions for patients and support systems across inpatient and outpatient populations. Collaborate with patients, families, and the Inter/Multi-Disciplinary Team to develop and implement evidence-informed, person-centered care plans. Advocate for patients while respecting self-determination, reducing health disparities, and adhering to the NASW code of ethics and Standards of Practice. Work closely with various departments throughout the medical center to support the care continuum. This combined posting considers candidates with either an LCSW or MSW license; successful applicants will be placed on a wage range determined by relevant experience and applicable factors.
Required Qualifications
- Master's Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
- Oregon Clinical Social Worker License upon hire
- License in states wherever care is delivered upon hire
- 2 years - Relevant Social Work experience. May include CSWA experience
- Master's Degree Social Worker
- Within 60 days of hire Oregon Clinical Social Work Associate License
Desired Qualifications
- 1 year - All EMPL's: Care Management: Experience in acute care settings
- 2 years Clinical work experience within healthcare, hospice and/or bereavement
