Job Summary
Provide socialization and stimulation to residents in a long-term care setting by conducting psychosocial groups, assisting with crisis intervention, and supporting community appointments. Monitor facility smoking programs, aid CNA/Nursing staff during meals, and manage personal items and contraband. Deliver 1:1 sessions to promote self-maintenance and group attendance while maintaining accurate documentation of all activities. Enforce Abuse and Neglect policies and ensure HIPAA compliance. Participate in interdisciplinary treatment planning, program development, and in-service training. Requires minimum one year experience in skilled or long-term care, CNA certification preferred, and ability to lift objects and residents over 50 pounds.
