Job Summary
Conduct psychosocial interventions, assessments, and supportive counseling for patients and families across hospice inpatient units, long-term care facilities, and home settings. Manage caseloads by assisting with advanced directives, funeral planning, and complex case management while evaluating financial barriers and facilitating referrals to internal and external resources. Provide short-term individual counseling on grief, loss, and coping; perform crisis intervention; and collaborate with interdisciplinary groups to coordinate care and revise care plans. Evaluate patient response to interventions and communicate psychosocial information to new care providers during transfers. Participate in on-call rotations as needed.
Required Qualifications
- Masters of Social Work (MSW)
- Master's Degree in Social Work from an accredited school of Social Work
- three (3) years direct service experience
- Experience in supportive counseling in group and individual settings
- direct service with clients and their families in health care setting
- Experience in provision of case management services
- Principles, practices and methods of psychosocial services interventions, case management. individual assessment. and counseling techniques (including but not limited to grief, loss and bereavement)
- Principles, practices and methods reporting and referral
- Current State of Florida driver's license
- driving record acceptable to Community Hospice insurance carrier
- reliable transportation
