Social Worker

Consult medical providers and staff on behavioral health interventions as a member of the Integrated Behavioral Health team. Facilitate internal and external referrals for specialized care through brief, solution-focused interventions to expand patient access. Provide consultative support and guidance while maintaining a full-time, permanent schedule with Monday through Friday day shift clinic hours.
