Job Summary
Social Worker in inpatient care management provides psychosocial assessments and direct social work services to patients in the inpatient medical and surgical units. Responsibilities include offering psychosocial support to patients and families, collaborating with the multidisciplinary team to develop and implement safe discharge plans, coordinating care for patients with complex social and medical histories, delivering case management and counseling in specialized programs, and making timely referrals to hospital and community agencies, including mandatory reporting when required. Per diem daytime role with emphasis on independent decision-making and health sciences teaching activities may be involved.
Required Qualifications
- Master of Social Work Degree from a program accredited by the Council on Social Work Education
- One year of full-time social work experience in a health care setting or equivalent (including practicum placement)
