Job Summary
Social Worker role focusing on care management for patients with chronic conditions or complex needs. Collaborates with physicians and healthcare team to assess and educate patients and caregivers, facilitates communication, coordinates services, identifies barriers, and supports optimal resource use while balancing clinical quality and cost. Responsibilities include psychosocial screening, medical record review, data gathering, developing/updating care plans, coordinating with external agencies, ensuring timely consultations and testing, intervening to resolve barriers, and applying DSM-5 guidelines for mental, emotional, and behavioral concerns. Works remotely with a 24/7 shift pattern including night and weekend coverage; engages in cost-benefit analyses and supports a comprehensive benefits package. Qualifying criteria include an MSW, active CSW/LCSW licensure, strong communication, and ability to work independently; preferred Case Management Certification and healthcare-care management knowledge. This is a remote, non-exempt role with hourly pay range $38.77–$59.82, fully remote in the U.S., with a salary range annualized from the stated hourly rate.
Required Qualifications
- Master's Degree in Social Work (MSW) from an accredited institution
- Current State Licensure as a Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW)
- Basic computer skills and knowledge of word processing and spreadsheet software
- Strong written and verbal communication skills
- Demonstrated ability to exercise critical thinking skills
- Preferred Qualifications Case Management Certification
- Experience working in clinical care management, social work or with third party payers
- Knowledge of care management and health insurance related products
- Working understanding of coding, episode of care and length of stay guidelines
- Ability to work independently, be self-motivated, flexible in a rapidly changing environment
