Social Worker II- Weekends/Part Time

Provide psychosocial assessments, treatment planning, therapeutic interventions, and complex discharge planning for designated psychiatric patients. Execute crisis intervention and coordinate referrals and resources to assist patients and families transitioning to the next level of care. Evaluate and implement treatment regimes in collaboration with case managers, physicians, and healthcare team members under the direction of the case management director. Ensure strict adherence to patient rights, privacy, safety, confidentiality, and the Baker Act. This part-time weekend role supports the social services team in managing the patient population.
