Job Summary
The Social Worker II provides social work services across patient groups affected by illness or disability, applying utilization criteria and case management principles within a healthcare setting, and leveraging community resources, insurance, regulatory, and legal knowledge. Responsibilities include psychosocial assessments, individual and family therapy support, advocacy, interprofessional collaboration, and rotating coverage as needed; requires MSW and relevant certifications with strong communication skills and familiarity with DBT/CBT and motivational interviewing.
Required Qualifications
- Master's degree in Social Work (MSW)
- BLS Certification through American Heart Association
- Completion of endorsed patient/visitor de-escalation and restraint program certification
- Eligible for rostering in preparation for licensure
- Two years of social work and medical crisis intervention experience
- Experience in individual and family therapy
- Five years' experience working in a healthcare setting (preferred)
- NASW code of conduct principles
- Psychosocial assessments, utilization and case management principles
- Group theory and dynamics knowledge with experience facilitating psychosocial and skill-building groups
- Individual and family supportive advocacy
- Age-specific competencies
- Interdisciplinary collaboration
- Excellent verbal and written communication skills
- Basic Microsoft Windows desktop application and navigation skills
- Strong communication and listening skills
- Knowledge of community resources
- Excellent collaboration skills with providers, legal representation, regulatory agencies and risk/compliance
- Willingness and ability to provide coverage on rotational basis
- Knowledge of Best Practice, DBT, CBT, and motivational interviewing
- Employee assistance in-services and consultation to staff, physicians, and community providers about psychosocial aspects of care
