Social Worker II - Optional

Social Worker II provides psychosocial assessment and develops a comprehensive plan for interventions and complex discharges for high-risk patients and families. Serves on multidisciplinary teams to optimize health status and smooth transitions between levels of care, offers education to promote the practice of social work within the hospital, and supports specialty areas requiring an MSW for regulatory/grant funding. Responsibilities include applying root-cause analysis to readmissions, coordinating with care teams and families, counseling caregivers, developing and revising care plans, linking to community resources, and providing on-call support per departmental policy. The role emphasizes collaboration, communication, education, crisis intervention, and preventive social work within a hospital setting.
