Job Summary

The Social Worker II will manage patient care, drive discharge planning, resolve psycho-social barriers, and support discharge needs, while Utilization Management reviews patient status for appropriateness and payer coverage. Requires a Master’s degree in Social Work (MSW or MSS) and state licensure (CSW/MSW) prior to hire; acute care social work experience is preferred.
