Social Worker II, Manhattan

Provide intensive case management and therapeutic support to a caseload of high-need residents by conducting psychosocial assessments, developing individualized service plans, and overseeing their implementation. Offer crisis intervention, mental health support, and linkage to substance use treatment while assisting residents in accessing entitlements, medical care, home care, and community resources. Facilitate group sessions to promote wellness, socialization, and skill-building, and serve as a resource for complex case consultations and multidisciplinary care planning. This full-time temp-to-perm role in Manhattan supports formerly homeless senior residents with trauma-informed, person-centered care within our housing programs.
